What exactly does Evercare do?

Evercare provides ongoing care coordination for seniors managing multiple appointments, medications, and insurance requirements.

We help organize appointments, track follow-ups, monitor medication logistics, assist with paperwork, and coordinate communication between doctors, pharmacies, and family members.

For eligible seniors, this type of ongoing coordination is supported under Medicare’s Chronic Care Management (CCM) and Advanced Primary Care Management (APCM) programs, which recognize the importance of non-face-to-face coordination between visits.
Do you replace my parent’s primary care doctor?

No. Primary care providers diagnose conditions, prescribe medications, and manage treatment plans.

Evercare complements medical care by handling the coordination around it — scheduling appointments, organizing documentation, tracking follow-ups, and helping families stay aligned between visits.

Under Medicare CCM (42 CFR §410.26), care coordination services are intended to support patients with two or more chronic conditions by managing care between visits.

Evercare operates within that coordination framework and does not replace medical decision-making.
How is this different from hiring a caregiver?

A caregiver provides hands-on assistance with daily living activities such as bathing, dressing, or meal preparation.

Evercare does not provide in-home personal care. Instead, we coordinate the broader healthcare picture — appointments, medications, referrals, paperwork, and communication between providers and family members.

We handle the logistics that often become overwhelming for families managing complex care.
How is this different from a hospital case manager?

Hospital case managers typically assist during a hospital stay or discharge process. Evercare provides ongoing, longitudinal coordination — not limited to a single visit or event.

We stay involved between appointments and across providers to ensure continuity.Medicare’s CCM and APCM programs specifically recognize the need for continuous care coordination outside of acute hospital settings.
Is my data secure with Prospect?
